Complex Care At Home support for Local Authority Commissioners

Safe, stable complex care packages at home

We work with local authority commissioning teams and social care professionals to provide safe, well-governed packages for people with high needs and complex risks, where a standard personal care service is not enough.

The pressures you are managing on the ground.

People’s needs in the community are becoming more complex. Multiple long-term conditions, polypharmacy, high-risk clinical procedures and safeguarding complexity are no longer unusual in community care commissioning. And the number of providers equipped to manage those needs safely, with real nurse oversight and clinical governance, remains limited.

When a standard domiciliary service cannot safely meet a person’s needs, the options narrow quickly. Residential care is expensive and often not what the person wants. Hospital is not appropriate if it can be avoided. And a package that breaks down through inadequate clinical oversight carries real consequences, for the person, for the family, and for the commissioning team.

Assessment-led packages designed for high-need, high-risk cases.

Every CareLink Direct package starts with a proper clinical assessment. We do not apply standard care models to complex situations.

We assess the individual, the clinical tasks required, the risk profile, the home environment and the family’s capacity, and we design a package around what we find.

  • Nurse-led assessment of clinical needs, risk and home environment
  • Care plans and risk assessments designed for complex cases, not adapted from standard templates
  • Staffing model matched to the level of clinical need
  • Contingency planning for staffing disruption, emergencies and deterioration
  • Documentation and digital records that are audit-ready
  • Regular clinical review to ensure the package remains safe as needs change

Complex clinical procedures at home.

Here is how CareLink Direct supports local authority commissioning.

CareLink Direct provides an alternative. Packages that are properly assessed, clinically supervised, contingency-planned and documented, for the cases where most agencies cannot help.

What We Do

CareLink Direct is a nurse-led complex care provider serving Sussex and Surrey, registered with the CQC for Treatment of Disease, Disorder or Injury

Complex clinical procedures safely supervised in the community.

Some packages require clinical procedures that personal care only services cannot provide. CareLink Direct is CQC registered for TDDI and is able to deliver nurse-supervised clinical care at home, including the procedures listed below. Each is delivered by trained carers signed off on specific competencies, with nurses actively involved in oversight and review.

  • Catheter care, urethral and suprapubic
  • Bowel care, including digital rectal examination and manual evacuation
  • Stoma care
  • Enteral and PEG feeding
  • Tracheostomy care and suctioning
  • Ventilation support
  • Vital signs monitoring and NEWS scoring
  • Seizure management and complex medication support

Safeguarding, MCA and DoLS

For packages involving people with complex safeguarding situations, those who may lack capacity, or those subject to deprivation of liberty, the commissioning team needs a provider that can demonstrate genuine competence, not just a policy document.

CareLink Direct staff are trained in safeguarding, the Mental Capacity Act and Deprivation of Liberty Safeguards as part of their core preparation. Our nurses are involved in best-interest decision-making where relevant, and we work with local authority safeguarding teams and social workers as part of the wider multi-disciplinary arrangement.

  • Safeguarding training embedded in staff preparation, not just induction
  • MCA and DoLS awareness applied in practice with nurse oversight
  • Involvement in best-interest processes where clinically relevant
  • Clear incident reporting and escalation procedures
  • Transparent communication with LA safeguarding teams when concerns arise

Refer, enquire or invite us to tender.

Contact us by phone, email or the referral form. We will acknowledge your enquiry promptly and confirm whether the package is within our scope.

Common questions from commissioning teams.

Yes. Our staff are trained in safeguarding as part of their core preparation and our nurses are involved in cases with higher safeguarding risk. We work with local authority safeguarding teams and communicate clearly when concerns arise.

Yes. CareLink Direct staff are trained in the Mental Capacity Act and Deprivation of Liberty Safeguards. Our nurses are involved in best-interest decision-making where relevant and we work alongside social workers and other professionals in these cases.

We operate across Sussex and Surrey, based in Worthing, including Brighton, Chichester and Crawley. We are expanding into Surrey and already support people there. Please contact us to confirm coverage for a specific area.

Because we have nurses and trained carers in place, we can often mobilise more quickly than providers who need to recruit and train. We will give you an honest timeline at the point of referral and will not commit to a date we cannot meet.

Getting in touch doesn’t commit you to anything. It’s just a conversation.

Our clinical team will listen to your situation, explain what we can offer, and be honest about whether we are the right fit.
