Elderly Care At Home

Staying home as you get older, with the right clinical support around you.

Getting older sometimes brings health changes that make daily life more difficult and staying at home feel less certain. See how we support elderley people at home

Growing older does not have to mean leaving home.

For most people, home is where they want to be. Their own surroundings, their own routine, the things around them that are familiar. That matters enormously, and it is something we think is worth protecting.

But as health needs become more complex, staying at home safely becomes harder without the right support in place. A new diagnosis, a hospital admission, a fall, a gradual decline in mobility or memory, a condition that has now reached a point where more clinical oversight is needed, any of these can make families feel that a care home is the only option, even when their loved one has no wish to go.

Here is how we support older adults and their families at home.

CareLink Direct is registered with the Care Quality Commission for Treatment of Disease, Disorder or Injury.

This is a regulated activity covering complex clinical care at home, and it means we can safely provide care that most agencies cannot. Our nurses are on the ground, involved in every assessment, and present in the care of every complex package.

  • Nurse-led assessment and care planning from the beginning
  • Clinical procedures carried out by trained carers under direct nurse supervision
  • Coordination with GPs, hospital teams, district nurses and specialists
  • Vital signs monitoring so changes are noticed and acted on early
  • Medication support, managed safely and documented correctly
  • Regular care plan reviews so the package adjusts as needs change

Safely delivered by nurses &, Trained Carers

Many of the people CareLink Direct supports are living with conditions that become more common as we age, conditions that may have been manageable for years but have now reached a point where day-to-day care and clinical oversight are needed at home.

Consistent, familiar carers who earn trust over time.

One of the things that matters most to older adults, and to the families who love them is seeing the same faces. A consistent team, who know the person’s routine, their preferences, and what a normal day looks like for them. Carers who notice when something is slightly different, because they know what it usually looks like.

How it works

Common questions about elderly care at home.

Standard home care agencies are registered to provide personal care, covering help with washing, dressing and meals. CareLink Direct is also registered for Treatment of Disease, Disorder or Injury, which covers complex clinical care at home. This means we can safely manage clinical procedures and higher-acuity needs that standard agencies cannot.

Yes. We provide care for older adults living with dementia, including support with personal care, medication, daily routine and safety monitoring. Nurse oversight means that clinical changes are spotted and escalated appropriately. Please see our dementia care at home offering for more details.

This is very common and there is no easy answer. We find that the way care is introduced matters as much as the care itself. We are experienced in working with older adults who are reluctant, and we take the time to build trust at their pace rather than imposing a routine. If you would like to talk through the specific situation, please call us.

Yes. We work with hospital discharge teams and families to arrange care at home following a period in hospital. Because we have nurses and trained carers ready, we can often begin care quickly once the assessment has been completed and the plan agreed.

Care plans at CareLink Direct are reviewed regularly and updated as needs change. You will not be locked into an arrangement that no longer fits. If needs increase significantly, we discuss the options with the family and adjust the package accordingly.

We are based in Worthing and serve the whole of Sussex and Surrey, including Brighton, Chichester and Crawley, within approximately a 40-mile radius. Please contact us if you are unsure whether we cover your area.

Care can be funded privately, through NHS Continuing Healthcare if your loved one meets the eligibility criteria, or through a local authority package. If you are not sure which applies to your situation, we can help you understand the options when you contact us.

Getting in touch doesn’t commit you to anything. It’s just a conversation.

Our clinical team will listen to your situation, explain what we can offer, and be honest about whether we are the right fit.
