Complex Care At Home for Children and Young People

Making it possible for more children with complex care needs to stay at home

Specialist nurse-led complex care for children and young people with serious or complex health needs across Sussex and Surrey.

Caring for a child with complex health needs at home is one of the hardest things a family can face.

Managing clinical procedures at home. Coordinating between hospital teams, community nurses, school, social care and the people who provide day-to-day support. Carrying the anxiety of knowing that something could change quickly, and not being sure whether you would catch it in time.

Many of the families we work with have been doing this, largely alone, for months or years before they contact us. Some were sent home from hospital with a care plan and limited guidance. Some have watched their child’s needs increase gradually, and found that the support around them has not kept pace.

Safe clinical care at home

CareLink Direct is registered with the Care Quality Commission for Treatment of Disease, Disorder or Injury.

Every care package starts with a nurse-led assessment of your child’s specific needs, not a generic paediatric care model. Our nurses are on the ground, involved in assessments, and present in the delivery of care. When something changes, someone clinical notices and acts.

  • Nurse-led assessment following individual referral
  • Care plans built around the child and the family, not around a standard template
  • Trained carers delivering clinical procedures under direct nurse supervision
  • Regular review as your child’s needs change over time
  • Contingency planning so care remains reliable in unexpected situations

Clinical procedures we provide for children at home

Children may need support with some or all of the following. Every procedure is delivered by trained carers working under nurse-led supervision, following individual clinical assessment.

What is children’s complex care at home?

Children’s complex care at home means specialist nursing and healthcare support delivered in a child’s home, school or community setting. It covers the clinical procedures and nursing oversight that allow children with serious or long-term health conditions to live at home and be part of everyday family life, rather than remaining in hospital.

How it works

Common questions from families.

Yes. CareLink Direct supports children from birth. Very young children with complex health needs require particular care and attention to dignity and safeguarding, and our approach reflects this throughout.

Care plans at CareLink Direct are reviewed regularly and updated as your child’s needs change, whether that means increasing support, reducing it as they develop, or adapting to new clinical needs. We do not lock you into an arrangement that no longer fits.

We work collaboratively with the full team around your child. We attend clinical reviews where appropriate, communicate clearly with paediatric consultants, community nurses and therapists, and ensure that care delivered at home is consistent with the wider clinical plan.

We are based in Worthing and serve Sussex and Surrey, including Brighton, Chichester and Crawley, within approximately a 40-mile radius. We are expanding into Surrey and already support children there. Please contact us if you are not sure whether we cover your area.

Getting in touch doesn’t commit you to anything. It’s just a conversation.

Our clinical team will listen to your situation, explain what we can offer, and be honest about whether we are the right fit.
