What is palliative and end of life care?

Palliative care is a form of specialist care that focuses on improving quality of life for people living with a serious, life-limiting illness. Rather than aiming to cure the condition, palliative care is concerned with relieving symptoms, managing pain, and supporting the emotional and practical needs of the person and their family.

What is end of life care?

End of life care is palliative care at the stage when someone is expected to die within the coming hours, days, weeks or months. It involves ensuring the person is as comfortable as possible, that their wishes are understood and respected, and that the people around them are supported.

Both palliative and end of life care can be provided at home, and for many people this is where they want to be. CareLink Direct works alongside GPs, district nurses, hospice teams and specialist palliative nurses to support people and their families through this time.

When is palliative care at home the right choice?

Palliative care at home is an option when someone has a life-limiting illness and wishes to be cared for at home, either throughout their illness or in their final weeks and days.

Families often consider palliative home care when:

  • A loved one has expressed a clear wish to remain at home
  • Clinical needs are increasing and the family needs professional support alongside them
  • Hospital or hospice admission is not what the person wants
  • Symptom management, pain relief and comfort need to be properly overseen
  • The family needs time to be present as a family, not just as carers

What palliative and end of life care involves

Our palliative care packages are built around the individual’s wishes and clinical needs. They typically include:

  • Personal care support, including washing, dressing and oral hygiene
  • Comfort and repositioning to prevent pressure sores and maintain dignity
  • Medication support and monitoring as prescribed
  • Vital signs monitoring where this forms part of the clinical plan
  • Communication with health professionals about changing needs
  • Waking night or sleeper night care for those who need overnight presence
  • Support with nutrition and hydration where the person is still eating or drinking
  • Practical support for the family and household
