Complex Care At Home support for Case Managers

Clinically robust, defensible community care for catastrophic injury and clinical negligence cases.

We work with case managers, solicitors, insurers and medico-legal professionals to deliver long-term community care packages for individuals with catastrophic injuries or complex clinical negligence cases, providing the clinical rigour, governance and documentation that these cases require.

The challenge at the centre of every catastrophic injury case.

Moving someone from hospital or inpatient rehabilitation into community-based care is one of the most clinically and logistically complex transitions in case management.

The individual may have life-long, high-acuity needs. The procedures required may be ones that most home care providers cannot safely deliver.

The documentation must be rigorous enough to withstand scrutiny in a legal or insurance context and the package must be stable enough to last.

Nurse-led clinical care for complex and high-acuity community packages.

CareLink Direct is registered with the CQC for Treatment of Disease, Disorder or Injury. Our founders are ITU-trained nurses with around 40 years of combined clinical experience between them.

Every care package is built following individual assessment. Every clinical procedure is delivered by carers who are specifically trained for that task, supervised by our nursing team, and signed off on competency. The package is designed to be safe, stable and sustainable over time.

  • ITU-trained nurses involved directly in care assessment and delivery
  • Carers trained in specific procedures before working independently
  • Nurse oversight maintained throughout the life of the package
  • Care plans and risk assessments built for complex, high-acuity cases
  • Regular clinical review and documentation
  • Contingency planning for staffing disruption, emergencies and deterioration
Delivering skilled nursing care in the comfort of home

Complex clinical procedures at home.

Clinical capability across the full range of catastrophic injury procedures.

CareLink Direct supports the clinical procedures commonly required in catastrophic injury and clinical negligence care packages, including the following. Each is delivered under nurse supervision following individual assessment.

Multi-disciplinary collaboration across the full case management team.

CareLink Direct is experienced in working as part of a wider team around a catastrophic injury client. We understand that case managers, solicitors, medical experts, therapists, insurers and the clinical team all need to work in alignment, and that the community care provider must be able to communicate clearly and professionally with all of them.

Cross Sector Collaboration

We collaborate with the full range of professionals involved in these cases and we maintain clear, professional communication with the case management team throughout. Our clinical team is available for case conferences, expert discussions and written reporting where required.

  • Established working relationships with case managers, solicitors and insurers
  • Participation in multi-disciplinary meetings and case conferences
  • Written reporting on clinical progress, care delivery and any concerns
  • Coordination with medical experts, therapists, hospital consultants and GPs
  • Professional communication throughout, suitable for a medico-legal context

Structured care plans

CareLink Direct maintains structured care plans, detailed risk assessments, digital records with real-time updates and clear audit trails. Incidents are reported and managed through a formal process. Our governance structures are designed to produce documentation that is transparent, consistent and audit-ready.

  • Structured care plans and detailed risk assessments as standard
  • Digital records and real-time clinical documentation
  • Clear audit trails for all clinical interventions
  • Incident reporting and formal investigation processes
  • Safeguarding procedures and MCA awareness applied in practice
  • DoLS considerations managed appropriately where relevant
  • Documentation suitable for use in medico-legal and insurance contexts

How to refer or begin a case.

Contact our clinical team directly by phone or email, or use the referral form. Share the broad clinical picture and your timeline. We will respond promptly and confirm whether the case falls within our clinical scope and geographic coverage.

Registered for TDDI

“Registered to provide Treatment of Disease, Disorder or Injury (TDDI)”, meaning we are approved to deliver complex clinical care at home for people living with serious illness, disability, or injury.

Common questions from case managers and legal teams.

Yes. CareLink Direct is registered with the Care Quality Commission for Treatment of Disease, Disorder or Injury, covering the complex clinical procedures typically required in catastrophic injury care packages. Our registration details are available on request.

We support a full range of complex clinical procedures including ventilation support, tracheostomy care, suctioning, enteral and PEG feeding, catheter care, bowel care, stoma care, vital signs monitoring, seizure management, brain injury care and palliative care. If you are not sure whether a specific procedure falls within our scope, please contact us.

We operate across Sussex and Surrey, based in Worthing, including Brighton, Chichester and Crawley, within approximately a 40-mile radius. We are expanding into Surrey and already support clients there. Please contact us to confirm coverage for a specific location.

Mobilisation timelines depend on the complexity of the package and the procedures involved. Because we have qualified nurses and trained carers in place, we can often move more quickly than providers who need to recruit and train from scratch. We will give you an honest timeline at the outset and keep the case management team informed throughout.

Yes. We work with privately funded packages, litigation-funded packages, and packages funded through insurance arrangements. Please contact us to discuss the specifics of a case.

Getting in touch doesn’t commit you to anything. It’s just a conversation.

Our clinical team will listen to your situation, explain what we can offer, and be honest about whether we are the right fit.
