Dementia Care at Home

Dementia is not a single condition but a term describing a range of symptoms associated with a decline in brain function. The most common form is Alzheimer’s disease, but there are other types including vascular dementia, Lewy body dementia and frontotemporal dementia. Each type affects the person in different ways.

Common symptoms of dementia include memory loss, difficulty with language and communication, confusion about time and place, changes in mood and behaviour, and increasing difficulty with daily tasks. As dementia progresses, physical care needs also increase, and some people develop complex clinical needs alongside their cognitive symptoms.

One of the most important factors in good dementia care is consistency. Familiar faces, familiar routines, and carers who know and respond to the individual make a profound difference to how settled and safe the person feels. CareLink Direct takes continuity seriously.

When is specialist care at home the right choice?

You may be considering specialist dementia care at home if your loved one:

  • Has a diagnosis of dementia and needs daily support to remain safely at home
  • Has increasing care needs that the family cannot manage alone
  • Has complex health needs alongside their dementia, including clinical procedures
  • Becomes distressed or disoriented with unfamiliar carers and needs consistency
  • Is at risk of falls, wandering or medication errors without regular support

supporting people with dementia

How our care supports people and families

  • Consistent carers who build familiarity and trust over time
  • Care plans built around the individual’s life history, preferences and current stage of dementia
  • Support with daily routines in a calm, unhurried way
  • Medication support and monitoring to reduce the risk of errors
  • Safety monitoring including awareness of fall risk and changes in behaviour
  • Nurse oversight and clinical review when health or behaviour changes significantly
  • Regular communication with the family so nothing important goes unnoticed
  • Coordination with the memory service, GP and other health professionals

What support may include

Care packages are built around individual assessment. Depending on the person’s needs, support may include:

  • Personal care including washing, dressing and oral hygiene
  • Medication support and prompting
  • Meal preparation and nutrition support
  • Orientation and companionship throughout the day
  • Safety monitoring and fall prevention support
  • Support with attending GP or hospital appointments
  • Night care for those who are unsettled or at risk overnight
  • Respite care to support family carers

We are also here to support you as a family member. Living alongside someone with dementia is demanding, and you deserve support too.
